Lingual braces cost more for several reasons that are not accurately portrayed in this column:
1. Unlike the front of teeth that are all basically the same shape from person to person, the back of the teeth (lingual) are vastly different from person to person and even from side to side of the same person. Although we are talking millimeters in difference in shape it only takes a mm of difference in the final position of the teeth for the teeth to look crooked. Lingual braces must be custom-made by a lab to fit the individual they are going to be placed on. The customization by a lab takes many hours of technician work and is a very machine intense process resulting in typical lab fees for the orthodontist of about $2000 per arch of teeth (each arch is one jaw of teeth).
2. Working on the back of the teeth to adjust braces is extremely difficult and requires lots of experience to get good at doing it. You are paying for experience of the orthodontist too.
3. Appointments also take twice or more time than braces on the front of the teeth and all orthodontists have to sell is there limited time in any given day so again, more cost.
4. Because of the difficulty of working on the appliances assistants can't do much of the work as is done with labial (front) braces and so more of the orthodontists time is required.
5. Orthodontic residencies don't teach much lingual orthodontics and so the ortho must learn it on his own requiring more time, travel expenses, course fees, etc... which must get reimbursed somehow--again, more cost to patients.
6. Very few orthodontists are willing to do all of the above and so supply and demand curve shifts again--so more cost to patient.
But if you want lingual treatment you can get everything finished well and to at least the same quality of finished results as any and all other types of braces. Invisalign cannot boast this and is a very limited in use and quality of finish (one ortho. study shows Invisalign gets 40% of the quality of result as standard fixed braces when compared using board standard measurements).
